12 month: Hib, Hgb, Lead
15 month: MMR, IPV
18 month: DTaP, (PCV7 - deferred secondary to national shortage)
24 month: Hgb, Lead, Varicella
30 month: none
3 year: Hgb, Lead
4 year: Hgb, Lead, UA, BP
5 year: DTaP, IPV, MMR, Hgb, Chol, UA, BP
7, 8, 9, 11, 13,
14, 15, 16, & 17 year:
UA, BP, Vision Testing
10 year: Td, UA, BP, Vision Testing
All females over 12 years: Hgb
Abbreviations:
Hep B: Hepatitis B vaccine
DTaP: Diphtheria, Tetanus, and acellular pertussis vaccine
IPV: Polio vaccine
Hib: Haemophilis influenzae type b vaccine (cause of meningitis in children)
PCV7: Pneumococcal vaccine (main cause of ear infections and pneumonia in children)
Varicella: chicken pox vaccine
Hgb: hemoglobin (to look for anemia, done by fingerstick)
MMR: Measles, Mumps, and Rubella vaccine
UA: urinalysis (urine test to look for infection or kidney problems)
Chol: cholesterol level
BP: blood pressure check
Td: Tetanus booster (given every ten year as needed after 10 years old)
